To register a educatee to nourish any district school, please phone call our Central Registration Office between the hours of 8:30 AM and 3:00 PM at 716-483-4344.The Central Registration Office is located in our fundamental assistants building at 197 Martin Route. Required Documents for Student Registration
Please bring all completed registration forms with y'all forth with the post-obit documents:
- Proof of Residence (1 of the post-obit):
- residential lease agreement
- home mortgage argument or tax bill
- signed argument of residence from landlord or property owner (also bachelor in Castilian)
Other documents may be considered if the above are not available. Please see the registration packet for examples, or talk over the options with the registrar.
If unable to provide proof of residence, delight complete the Supplemental Residency Questionnaire provided by the registrar.- Proof of Pupil's Age (one of the following)
- certified birth certificate
- certified baptismal certificate
Other documents may be considered if the higher up are not bachelor. Please run into the registration bundle for examples, or discuss the options with the registrar.
- Wellness and Medical Information, including:
- immunization records, issued past doctor or clinic
- record of physical examination completed within the last year, if available
- The Dental Wellness Document class should be completed by the parent or guardian and the dental role at the time of the child'south checkup. The form may be faxed to Pupil Services (716-483-4278) or brought to the schoolhouse after the checkup.
- The following information may as well exist requested, if applicable:
- affirmation of parental relationship
- report carte du jour or transcript from previous school
- any other wellness, psychological, or other pertinent evaluations

If your child has an Individualized Education Plan (IEP) or 504 Program:
